Your First Visit

Please download/read/ fill out all the attachments below. Please complete this prior to your initial visit to the office. Once completed, please bring all the information with you to your first visit. Do not wait to fill this out in the waiting room. If you do not have these forms available at your appointment time, your appointment may be canceled. The forms will take approximately an hour to complete. PLEASE ARRIVE TO YOUR FIRST APPOINTMENT THIRTY MINUTES AHEAD OF SCHEDULED TIME AND WITH ALL THE FORMS FILLED OUT.

If your appointment is thru telemedicine, the forms will have to be submitted 72 hours in advance to the clinic.
