Please download\/read\/ fill out all the attachments forms on the side. Please complete this prior to your initial visit to the office. Once completed, please send all the information back as soon as possible. If the forms are not submitted in timely manner, the appointment will be cancelled. Do not wait to fill the forms out in the waiting room. If you do not have these forms available at your appointment time, your appointment may be canceled. The forms will take approximately an hour to complete. PLEASE ARRIVE TO YOUR FIRST APPOINTMENT THIRTY MINUTES AHEAD OF SCHEDULED TIME AND WITH ALL THE FORMS FILLED OUT.<\/p>\n

If your appointment is through telemedicine, the forms will have to be submitted 72 hours in advance to the clinic.<\/p>\n
