New Patients

If you are interested in an appointment please fill out the NEW PATIENT INTAKE FORM and send it to the clinic via fax/ email/ or in person. You may also have your doctor or therapist refer you to the clinic. Please fill it out completely. If approved, you will be notified within 30 days (in most cases) about your appointment. You will then be scheduled for an appointment. (The scheduling of appointment varies). Appointment availability and approval are dependent on a variety of factors, including the availability of the medical practitioner.

We do offer telemedicine appointments for residents of Louisiana.

If this is an emergency please call 911 or go to the nearest emergency room. We are not an emergent care facility and we do not accept walk-ins.
